Contact numbers are provided above. They are for our CEO, COO, and VPO. You may also contact us at the provided email. We will answer all your questions and discuss needs and expectations you may have. We want you to be knowledgeable and compatible with our practice.
A site visit will occur before any scheduling of anesthesia. Prior to any scheduled anesthesia days we will need a contact form for each patient preferably with a single contact number. A health screening and history. A signed consent form.
Prior to Scheduling
Scheduling Requirements
To schedule a day with us we require a minimum of two patients. If you do not have two patients but require us to provide help we ask you to make out a separate check to our anesthesiologist of $5oo.
Day of Appointment
On the day of anesthesia we will arrive about 30 minutes prior to the patient in order to setup our equipment. We ask that your staff confirms the patient weight and NPO status.
After Procedure
At the end of the procedure we will provide your front office with the anesthesia time and they will do any necessary credit card processing through our bank machine/square/etc.. If care credit account is being used they will collect all the relevant information and identification. We will provide you with a copy of the anesthesia record at the end of the day as well as a copy of the bill.
